DOCUMENTATION TO BE DELIVERED WITHIN THE START OF THE MASTER

In the list below you will find the documents needed to be handle to POLI.design before the beginning of the master. If you encounter any problem, keep in mind that you can always write an e-mail at: formazione@polidesign.net

WHAT DO YOU HAVE TO DELIVER?

- Enrollment form sent by POLI.design Training Office together with the Private agreement in order to formalize the enrollment
- 16€ tax stamp (you can buy it in every "Tabaccheria" in Milan asking for "Marca da bollo")
- 1 passport photo
- Photocopy of the ID card (passport for UE/non-UE)

DICHIARAZIONE DI VALORE IN LOCO (DV)

This document is mandatory only for italians, EU and non-EU who have not achieved the University degree from an italian University

 

What is it?
The DV is an official document which provides a short description of your academic qualification, awarded by an institution belonging to an educational system other than the Italian one.

How can you get it?
It is a document you need to request from the Italian Embassy in your country of residence or graduation.
Depending on the country, the estimated time to achieve this document goes from 2 weeks until 3 months.

What do you have to deliver?
The Dichiarazione di Valore in loco (DV) must be delivered in original, together with (original or a legalized copy):
– the degree certificate (with the translations in Italian)
– transcript of exams (with the translations in Italian)

VISA & RESIDENCE PERMIT

This document is mandatory only for non-EU

 

What is it?
The visa is a document issued by the Italian Embassy in the country of residence, necessary for the regular entry into Italy.

How can you get it?
It is a document you need to request from the Italian Embassy in your country of residence.
Depending on the country, the estimated time to achieve this document goes from 2 weeks until 3 months.

Do not forget!
Once in Italy, you have 8 days of time to proceed with the request for a residence permit for study.
In order to apply for it, there is a “kit” available at all the post offices displaying the “Sportello Amico”.

CODICE FISCALE - TAX ID

This document is mandatory only for non-EU

 

What is it?
The Codice Fiscale is a tax identification number used in all interaction with government agencies and public administration in Italy. It is a plastic credit card-sized card with the unique 16 alphanumeric digit code devised from your’s name, date and place of birth.

How can I get it?
Application must be made at the office of the Agenzie delle Entrate.
Remember to bring with you the passport or (if you already have it) the Residence permit.
We suggest to go to the office in Milan downtown:

What do you have to deliver?
You are required to hand over a copy of the document that will be issued immediately by the Agenzia delle Entrate (it is the document will be issued as soon as you will apply for the Tax Id. Instead, the official Tax Id card will be sent by post at a later time).

Keep in mind

To formally enroll in the Master in Service Design, the most problematic documents are the visa and the Dichiarazione di Valore in loco (DV) both issued by the Embassy. Achieving these documents can require several weeks, sometimes months. Keep in mind that, in some cases, the Embassy wants to have the Dichiarazione di Valore in loco (DV) before proceeding with the visa.
